I created a book called "Contents" at the top level and then enabled Book Navigation ABB and got the block showing up as expected. I then edited the top level page of the book and changed the title to "By category". The block disappeared and I can't get it back. I've tried clearing cache, editing the title back to "Contents", creating new books, disabling and reenabling the block, update.php. Nothing works, I can't get the block to reappear.

I fixed my issue by creating a new block and using it instead of the default one. The default block seems permanently broken.

Completely uninstall ABB module then install 6.x-2.0-alpha1 and see if the bug still there. I believe the patch fixing similar issue is already incorporated into 2.0.
