Fix known issues of url, permission group creation/edit form.

Fixed drag handle in backend caused by bootstrap4-grid.css

• Add details to server template launch form updated 8 Feb 2019 at 20:44 PST
• Attach IAM role when I edit an Instance updated 8 Feb 2019 at 15:47 PST

• Fix bug when White Screen of Death happens when formatter has not been properly applied.
• Fix a bug so images can successfully be deleted
• Update to selector text input so it doesn't error when ajaxed in.
