Help - "Invalid Entry" error in user registration

If I try to modify an exisiting user (add a role or change a value) or if I try to register as a new user I get an "Invalid Entry" error and the data doesn't go into the database. I have checked the error log generated by the web server - there is nothing there.

Is anyone familiar with this issue and/or is there another place i can look for a more verbose error to track down the issue? Thanks!
BTW - this wasn't happening a few hours ago - perhaps there is a buggy module I have enabled?

Running (CivicSpace 0.8.1.3, Drupal 4.6.3) install.

Problems adding a second block.

I added one block just fine to my drupal site. Now I'm trying to add another, and I'm receiving an error.

The errors are listed on this image:

Any help would be appreciated...
