I have added all the necessary modules. I added an image field to the story node. I also added a preset (thumbnail/scale) to the imagecache. I applied the preset to the display of the image field in the story node. The image does not show up, but it is created on the file system. Instead of the image showing up, a small vertical gray bar shows up. Its about 2 pixels wide and 7-8 pixels tall.

This could be completely unrelated, but I believe that either clean URLs or private file system is required in order to use ImageCache.
