Can you confirm that the aliases are not created - they do not exist at all in your admin/build/path ?
Or is it just that the edit_term page is not finding and using it?

Suggestions:
admin/settings/pathauto
- Enable verbose feedback
- Check the Update action: Create a new alias. Leave the existing alias functioning.
... it's possible that pathauto may try to conflict with the manual setting. I'm not sure why or which one should win, but the option of choice is there in pathauto.

I think I can replicate a problem like, this, however it also happens after I disabled edit_term again, so I'm confused. Bulk generation, even after I emptied the path table totally - is not working for pathauto.
OK. ... on investigation it seems more to do with pathauto, and bulk updates over a certain number when only aliasing some of the vocabs. I'm not able to see my problems (a messy test database) as being directly related to edit_term. ...
Once I smoothed out my multi-thousand taxonomies, edit_term continued to work as expected.

Can you confirm that pathauto works as you expect when edit_term is not installed?

If there are more than [the number of allowed bulk updates] terms with no pathauto alias pattern set ... any further aliases are not generated. This appears to be a limitation of pathauto and the SQL lookup it uses.

I turned off edit_term - and pathauto became to generates aliases as expected!

@Eugef, same for me.

I also noticed that the Edit Term page didn't have the option to automatically generate alias.

@Benjamin Melançon, my issue (http://drupal.org/node/297749) is a duplicate of this one. so please mark mine accordingly. sorry for the mess!

if you have a new version, I can test it out for you. but please don't send me a patch, I can't manage them.

Current release includes a fix for paths going missing (was deleting aliases if a term was saved programatically)

This MAY probably fix this issue.
