Hello,
In our module we've released a few version after 3.45 (which is marked as recommended), but none of them are shown as "recommended" on the main page. How can we fix this and why it happened?

Probably a timing issue
Checked it and it looks ok now. Next time raise an issue in the webmaster project.

Sorry, misunderstood.
Thought the 3.45 was what was missing. Leaves my remark about putting an issue with the webmasters project. They'll pass it on the the appropriate people ( Customizations or Infrastructure) depending what is causing the problem.

If you go to the module project page, then click 'edit' -> 'recommended release', is the correct value selected?
If it is, it sounds like a bug. As MMJVB said, you should open up a ticket in the infrastructure queue for support: https://www.drupal.org/project/issues/infrastructure
