I created 2 new content types, 1 for images & 1 for a Color-box gallery. Everything was hunky-dory.
Then today I needed to add some pics to one of the galleries. When I tried to upload an image to a gallery, the uploader just 'hung,' that is, the progress spinner just keeps spinning & never seems to finish the upload. But, if I give it a little time & save the content, my new image appears, that is, the upload completes. If I choose to edit the image, it appears in the list of gallery images with the associated text fields (that's good).
Similar problem trying to delete existing image, except saving does not delete the image.
Same thing with the image content type: uploads appear to stall, but can be saved, but can't be deleted.
The images are well under the file size limit & I haven't altered any permissions on the server or changed memory limits.

temp workaround; still looking for answer
I have managed to locate the db entries for the unwanted images & delete them manually, along w/ the corresponding image files, but this breaks the user-maintainability of the site. Also, any time I flush the cache I see this in the logs:
“The file was not deleted, because it does not exist.” The time corresponds to when I flushed cache.
Solved
It turns out this is being caused by jQuery Update (7.x-2.x-dev). Disabling it restores image widget functionality. Reporting to issue queue.
Same behavior no JQuery update
I am using an image field set to store unlimited images......ater uploading about 230 images I get this behavior.....can't delete an image and to add one I must select the file then save the node....hitting the upload button causes the ajax blue spinner to go but file does not upload
Using Drupal 7.12

It has something to do white the google map installed. When I uninstalled that one all works fine. Hope it can help others.
Same error - ajax hangs on
Same error - ajax hangs on image upload/delete.
Solution - replace misc\jquery.form.js (ver. 2.52) with jquery.form.js 2.69 from \sites\all\modules\jquery_update\replace\misc\jquery.form.js

I had a similar problem. It is worth checking the browser console for any errors. In my case it was due to X-Frame-options setting on my web server. It was set to DENY, I had to change it to SAMEORIGIN and then the problem was solved.
