Unnecessary paging

I have a strange issue that I can not pin to a specific module and was hoping someone could point me in the right direction.

Basically, all comments offer a pager even when unnecessary on all content types.

So if I create a new article and then post a comment, there will be a pager showing 6 pages even though there is only one comment.

e.g. (1 2 3 4 5 6 next › last »)

All pager options bring you to new URL (i.e. page=1, page=2) but they are all actually the same page.

Help in finding a starting point to troubleshoot would be appreciated.
