Drupal is up and running but how do I ...?

All of a sudden: Access Denied. You are not authorized to access this page.

I have come across a most unusual problem. (for me anyway)

Our site has been up and running for some time now as you might note from the release version below. Unfortunately the heavy modifications made have led to it falling behind on the upgrades but even so we have had very little problems at all. User and abuser (xmlrpc) problems being the sum of them.

However over the last few days anonymous users have started getting the error "Access Denied. You are not authorized to access this page." Also we have started experiencing new mysql problems not least of which is huge load on the server and pages not being returned but errors such as: "Warning: mysql_connect(): Too many connections in /var/www/htdocs/drupal1/includes/database.mysql.inc on line 31 Too many connections".

To my knowledge and having questioned anybody else who might have access to the configuration no changes were made and the only 'out of the ordinary' occurance which might have precipitated the problem was a user/post being deleted - a spam troll.

I have been up and down (and up and down) the configuration and I can see no changes that might have accidently occured which could be restricting access.
I have searched the support archives for both the errors above but could not find anything relevant to our specific problem although I did check out the 'node privacy byrole module' and replaced it to no avail.

I have created a account for a user as a authenticated user. I want to stop this user changing there password and other setting. How do i do this. Can a get ride of the edit tap in the user account page?

Better looking node creation forms

I am looking for a way to make the node creation forms look better. Currently, they look hopelessly cluttered and my users who have never used a CMS before are having a hard time figuring things out despite a lot of help text that I wrote. A flexinode form with many text areas is particularly ugly.

I am specifically looking to do the following:

1.) Have the help text (form element description) appear on the left and the form element on the right
2.) Hide the 'Formatting Guidelines' stuff for text areas either completely or selectively.

Do I still need to appy the Location upgrade on a fresh install? I did and it still doesn't work....help please.

Automatically add nodes a menu (how)

I'm trying to make it as simple as possible to add content to specific areas of a website.

Currently I'm using flexinode and pathauto so that aliases are made (/) when content is added, and I was curious if there was already a way to also add that node to a menu of the same name as the flexinode.

I'm aware of how simple menu_otf makes it, but total automation would be nice :p I'm slowly learning Drupal, but I wanted to see if anyone had an ideas before I delved into the api. Any pointers or nudges in the right direction would be much appreciated <3
