Help needed from Drupal and MySQL expert

I need help for making my Drupal

My old Architecture: Drupal 6.x and MySQL database (Ver 14.14 Distrib 5.1.54) both Drupal and MySQL are in the same host.

Migrated to the new Architecture: The Sys Admin already made the migration

Server 1: Druapal 6.29
Server 2: MySQL version 5.1.69.

I was able to establish the connection between my server 1 (Drupal) and server 2 (the database server).

Server 1: I have the setting.php is located in /var/www/html/drupal/sites/default and /var/www/html/drupal/sites/mysites1.org

Proxy with Drupal

I'm try to explain shortly the my problems :(:
I have a Drupal 6 installation with apache and in front I have installed nginx how proxy.
Since I have put the proxy in front to Drupal I have problem of indexing of many pages of my site.
I would like if I must configure anything and what to work properly the my drupal ?? :(

My homepage is not showing some blocks .How to get back my original homepage

I am updating the Homepage of an old drupal website .It has lots of bocks in the homepage .Now the problem is that am not able to see some blocks like ( news section in my home page ) ? can anyone help me in this .

I created a php webpage in Drupal. It was working for a while and not working anymore. I found out the culprit is the form post "action". I get an error "Page not found" when loading the page. Below is the top part of my basic html form settings:
