The issue #483176: Duplicate entry errors caused by race condition, which #1710328: DOException: SQLSTATE[23000]: Integrity constraint violation: 1062 is probably a duplicate, would be enough to warrant a new beta release. After just installing for Flag support, we were surprised to have regular multiple PDO exceptions being thrown once enabled on a live site, only to find the issue resolved (hopefully) back in May.

i.e. Usage suggests there are either a lot of patched modules or sites susceptible to this error.

Marked critical as this is likely to be throwing irregular errors on the majority of sites that are using this module.

Random fact about having alpha / beta / rc releases, is that Drupal security team don't notify / action security issues.
