I just uploaded this, and when I went to install the module there's no name, version, anything. Just a blank check box. Am I missing something?

I can not replicate this bug. Everything appears to be working as expected. Please submit the version of drupal you are running, where you have placed the PCP module in your site directory and any other configuration details that would be important to help address the issue.

Closing Ticket. No one has reported the same issues and I can not replicate.
