This would be great usability feature:
If value is not in the list. This is important (at least for me) feature to have for the long and frequently updated taxonomy vocabularies. Add at the bottom of the form text field and the "Add..." button, that will add new value from this text field to both - list of available and selected options. I hope that it's doable.

This would be a great feature, and it's ommission is currently preventing me from using this module. Can anyone provide me with a patch, or even tips to write this in myself?

multiselect-6.x-1.x is no longer supported, because Drupal 6 is no longer supported.

Can you confirm if this is still needed in multiselect-2.0.0-beta4?
