The modal is not styled by my theme (Radix) and not scrollable.

All other modals (for example created by the 'modal' module, for node edit, node view, entityforms, etc.) are styled well by the theme, just the ones produced by this module are not replaced.

Maybe you apply some custom (weird) HTML/CSS markup?

I use the Bootstrap theme and encountered this issue but only on Firefox.

I noticed bootstrap theme adds a js file to override ctool modal and i think it might be broken. So i just added this line in my subtheme info file:

 exclude[js][] = 'sites/all/themes/bootstrap/js/modules/ctools/js/modal.js'

This seems to fix it. It now shows a scrollbar on longer forms. No ideal, but works.

This sounds like an issue of the bootstrap theme not this one. Maybe i'm wrong and feel free to reopen this issue.
