Installed the custom link as document suggests for use with Webform modul, modal window opens and then blank window with spinning wheel and nothing appears. Any suggestions.

Check your javascript error console. In my case a custom Drupal.behavior was running for a second time when the modal opens, throwing an error, and preventing the modal from loading the webform.
