Allow returning multiple results from the entity_lookup process plugin

The entity_lookup process plugin accepts multiple values, but it only ever returns a single matched entity. It would be great for multivalue fields if this could return all the matched entities.

Here's a patch that updates the plugin to return all matched entities; the patch also moves the destinationProperty mapping so that it happens after case-sensitive filtering happens.
