The first preset I made had both ()'s and a question mark. I tried viewing images and it failed. After playing around with it and some research, I discovered that the name must be alphanumeric and underscores.
There should be a note about this when creating the preset name. Ideally, it should also check to see if it matches those criteria and alert the user if it fails. If this had been in place imagecache would've worked flawlessly for me.
This is similar to this other issue: http://drupal.org/node/353539.

drewish commentedGood call, please roll a patch. Or suggest some sample help text.
drewish commentedare you sure you're using the latest version? I'm seeing "The namespace is used in URL's for images to tell imagecache how to process an image. Please only use alphanumic characters, underscores (_), and hyphens (-) for preset names." below the preset name. Please re-open if that's not the case.
