I don't quite understand how imagecache is triggered. Can it process images as users upload or insert them using the imce module, or must images be added using a cck field (or the core upload) in order for imagecache to be triggered?

Yes it can. For non-CCK images you'll need to pass them through the theme function.

Sorry, I'm not sure what that means.
Any docs or an example?

IMCE has capability to manipulate images on upload, but it is the user's choice.
I'm looking for a way to standardise these images automatically whilst still uploading via IMCE.

i'm really not sure how you'd go about doing that because i've not used IMCE. it might be worth opening a support request in that queue (or moving this one over there) and asking if they provide any imagecache integration.
