I can confirm this issue. The image class is not added on any Display Suite field template mode: default, minimal, reset, or expert.

Looking at why now. If anyone can provide suggestions, it would be very much appreciated!

I don't know if this is best, but since this module is awesome, but overall, very simple, I've added a request that Display Suite provide this same functionality. See the issue at: https://drupal.org/node/2222139.

Since Display Suite offers so many features and it really takes over field display (when templates are enabled), I think it might be better to merge this into DS. Your thoughts?

Sorry guys but I tested this with a freshly installed Drupal installations and everything was fine. Maybe this was fixed in Display Suite, I don't know :/

Just tested this, 7.x-1.0 does not work with DS but latest dev version works.
