Store the title of the current page with each feedback entry. Benefits:
- easier to identify on which page a feedback was given
- useful replacement token for #353548: Add trigger/action for e-mail notification.

The Drupal 6 version is no longer available, and no longer maintained. So I'm going to close this issue as outdated.

We do record the URL in the 3.x version, but I can see how it might be helpful to record the page title too. I'm re-opening this in the 3.x queue.
