Great module. Will prob use it during the testing of most of my upcoming sites.

I think it would be good to have the browser and OS info as part of the feedback report. If someone reports that something doesn't look right I would definitely want to know the browser.

Would you be interested in a patch for this? I think I would use the php get_browser() function: http://us3.php.net/function.get-browser and include it between the 'Location' and 'Date' on the form.
