User interface changes

New instance settings: FIELD prefix & suffix (empty by default)
New formatter setting: Display with FIELD prefix/suffix (FALSE by default)

API changes

The range field widget has been slightly modified in order to better reflect what we are doing. Might affect those who altered the widget in custom code.
