I'm using drupal 7.59 for my website. Now, text box for username filed is missing in user login page. So, I can't login into my site.

"Create new account" tab is lost in user login page and under "Request new password" tab, "username or email address" text box is missing too.

Then, also missing text box in "contact us" form which is created by webform. Someone please help me. I'm waiting for reply and advice.
