[meta] Templates and preprocess functions clean up

Bootstrap 7.x brings html5, mobile first, improved accessibility, etc to Drupal 7 markup.

Drupal 8 has almost all these items out-of-box.

Also lots of stuff was removed or simplified. So lets go through templates, preprocess and process functions and decide what can be removed at all or rewriten.
