Token Filter with custom token

I'm using token filter module, along with the token custom module. I can get default tokens to show in my text field, but not custom ones. What is the format for custom tokens?
In the "Token Filter" read me it states:
"Then in the text where you use that input filter you can use substitution tokens with
[token global site-name] etc. You can the global, user and any custom context available."

Maybe I'm typing something wrong? not sure the exact format for the custom token
