I found this module http://drupal.org/project/uc_webform_pane which seems like it should do exactly what I want. Does anyone know if there is anyway to make it so a small web form (survey) questionnaire can appear during the checkout of a product? This module says it allows you to ad a webform to a checkout pane in UberCart. The only thing that I've been able to do was get the form to appear within the product's node itself (above the add to cart button). Who wants to see a survey before they even add to cart haha? If anyone has done something like this, please give me any help or advice. To sum things up, I just need a way to get a webform to appear when someone is checking out.
