Bug fixes

• Created the br_address_field and storage to Customer Profile
• Created fields: br_birthdate, br_doc, br_name, br_phone on Customer Profile
• Created date format Brazilian only date.
• Removed commerce address field from form and view display mode
• Removed fields (cpf, birthdate, phone and doc) of the Payment Method that now are part of Customer Profile
• Uninstaling unecessary fields (cpf, birthdate, phone and doc) storage definition from Payment Method.
