Initial implementation of migrate source plugin for ical data.

This is a rewrite of the 7.x-1.x branch. All javascript has been moved to js files.
Another feature of this release is the fact that all ads are rendered only when they exist in the viewport. For more information please see Viewability best practices.

Initial production release.

Upgrade procedure

The following should do the trick in most cases, switching from previous alpha releases. Make sure that the destination is correct such that you overwrite the older version. Drush should ask you for confirmation when running the command. If it doesn't, change the destination to match wherever you have it installed already.
