Fixes compatibility w/ media module; adds setting to disallow duplicate file uploads.

Many issues have been fixed before releasing this new version.

Critical update : When clicking ajax links it will remove all classes. Now fixed so it will remove only 'active' class. To upgrade , replace existing module folder.
