Must select at least one category

I've installed and configured both the Yelp API and Yelp Blocks. When I attempt to create a Yelp Block and fill out all the fields on the set up form and Save, i get a status message indicating that I must select at least one category. I get that, but nowhere on the page is there a field to select the category.

Am I missing something?
