IMO this can only happen if you did not finish the workflow configuration. But a fatal error is not nice.

Workflow has been working great for months and now it happened again randomly. We have been pushing code all the time.

The test server has the same code and database and it worked fine. There is some differences in Pantheon config for live and test but nothing in code.

What does this mean? Any clue what this could refer to?

Adding a patch to fix the error, it appears the array could be empty and causing the error. I just check to see if its empty if so then I create the array so that it can be appended.
