No controls on add/edit form + no confirmation message

I think this may have happened on the most recent upgrade to 6.x-3.4. When I try to add or edit a webform, I only see the standard node controls -- title, body, input formats, etc -- none of the webform controls show up -- confirmation message, validation, etc.

Also, not sure if this is just something I'm doing wrong, but no confirmation message is shown when I submit the form. From the user's point of view, the form just reloads with all their data cleared, so they have no way of knowing if the submission was processed OK. I have verified that `confirmation` is set in the `webform` table.
