Use #field_prefix and #field_suffix instead of theming in file.inc

Now that Webform 3.x doesn't need to worry about Drupal 5, we should use use the convenient #field_prefix and #field_suffix properties on textfields where appropriate instead of using str_replace(). This patch makes two obvious wins in file.inc.
