Problem Encountered
Whenever a video is pulled up in the content it is attached to, the thumbnail attempts to be re-generated, even though an adequate copy already exists. See screenshot.
Note that the thumbnail name keeps advancing by +1 on the end of the sequence.
Expected Behavior
When a video is displayed or retrieved for the content it belongs to (for editing), an existing imagecached thumbnail will be used, or ... a new one will be generated.

The files folder and the video_thumbs folders are fully writable, so i'm not totally understanding when this happens.

Could you turn on "Log all executed commands to the Drupal log" and report the commands that are executed and the output?
