I am not bale to upload images!

Hi all
I have created my new theme, and while using this theme i ma not able to upload the images
Module simply accepts the input and just refresh the page.
It doesnot shows me the progress bar and not uploading the image.

But If i am using the Garland theme it allow me to upload image, it shows progress bar, and uploads the image.

Its the same case with server and my local machine.

I know i had left some thing in my tpl.php files.
di i? or i have to include some more lines anywhere other than .tpl.php files?
do I have to include some .js files?

I am suffering from this since last week, please advice me, am going crazy now.
