For my import, I'm imported a set of core and profile2 fields - including the core user picture field. When I import both in one CSV, groups are ignored.

Actually I've looked further into this, and I think there is a bug.

I have UIF and UIF Plus enabled and I'm importing a CSV with picture, role, and groups. If I try to import all three, groups are ignored. If I remove the picture column, groups are imported.
