Added Indipendent Table name.

Hi,

in the following patch you will found some minor change and a new feature.

• I've renamed the break . $i that were outside of the for in start_table end_table personally I found it more readable as the $i were meaningless.
• I've corrected the index of the break closing the rows break . ($i + 1)
• More Interesting: I've added a Name to the tables. This would print a span with the table Name just before the table div.
