I'm getting this error after I upgraded Drupal with the slash template

Fatal error: Call to undefined function: format_name() in /home/lordshaper/www/radiofurnace/themes/slash/slash.theme on line 144

I reuploaded the theme and it works now but I've lost all formating and login info on the site

The slash.theme file is part of the 4.6 version and older, not 4.7 or newer. I guess you by mistake downloaded the wrong version or did not properly clean out and old version you had since earlier.
