Also I've done a little code clean-up around the modified lines.

Hi guys, i have made some improvements and code refactoring of @yce's patch.

Most important changes:

- generated Salesforce's entity type should be selected on entityform's level, meaning i can have some entityforms that generate leads along with some entityforms that generate cases.
- added Salesforce Case URL, because they are for sure different from Lead URL
- added Record Type Case, because they can differ from Record Type Lead
- some code refactoring
