Change questionnaire_question_getquestions call

questionnaire_question_getquestions is used to determine which questions to

- $context (id)
- $context_entity_type
- $uid
- $amount = 0 (where 0 = all questions, and 1> equals the most important)

Also, add $amount to the function questionnaire_question_submission_form so that the form can display the correct amount of questions.
