Fatal error on add/edit (and menu move)

Hi there,

I'm looking at using this module on a older Drupal 5 installation. The D5 version has a fatal error when you try to add/edit a source. This patch fixes that.

Also the menu items are all at the top level of admin so the settings don't show on the admin page so I also moved them into the 'content' section and title cased them to match the D5 standard. I probably should have made these two separate patches but it's a pretty small patch so I figured I'd just do one.
