Remove deprecated code

There are some coding standards that I learned in 4.6 that were deprecated in 4.7 (see http://drupal.org/node/22218). Some of these (like print theme('page', $output) -- are minor. Some (like node revisions) are more critical.

The current release version of the module does not support the node_revision table.

Before porting to Drupal 5, I will need to update the core mysite module and plugins to follow all 4.7 standards.
