The code in MigrateDestinationFlagSimple is designed to work against the D6 version of flag. I will submit a patch for this issue to bring it up to date.

I think the patch in #2029613: Flag 3.0 Support is a little better - it supports both the 2.x and 3.x versions of flag and adds a Taxonomy handler. Also, the patch here contains some debug 'SAM:' strings.
