Release notes

Bugfix release. This cleans up a problem with menu entries when book module is enabled. After installing the updated module, you should run the Drupal upgrade script.

-- #526756: Duplicate entry errors on menu item update by nonsie and keinstein. Fixes duplicate entry error and book module handling.
-- Fixes reversed documentation of the API hook arguments.
-- Creates the 6.x.1.2 release.
-- Requires running update.php.
