I have had this issue following random spammage on the contact form.

I have upgraded to 6.12 and replaced the index.php file on several occasions.

I reset password before and after i upgraded to be extra careful and I still have this issue.

Within index.php at the bottom I find the lines:

I have tried removing and installing new index files

I have reinstalled site completely with new passwords....

help!

the index.module has nothing to do with the index.php in drupal core. I suggest you use a forums for this issue.
