mkdir creates directory control only in root, and only allows deletion

IMCE Mkdir is installed and active, and the admin profile we have created has both the permissions for creating and deleting a directory enabled. The settings also are set to the root in Directories (using a .) and the include subdirectories box is checked.

Unfortunately, the only control we get is the Directory icon seen in the screen capture. It will only appear when in the root, and not when in a subfolder. It also only allows for deletion, as shown in the screen capture.

This doesn't match the described behavior for IMCE Mkdir or the images provided in the documentation. Please let us know if there is something that we missed in the setup of the module.
