Since more often than not this cck field is used in conjunction with imagecache, I think the responsibility is probably more on the imagefield module to handle this bug:
#615418: image file with '+' sign or & in name not found by imagecache

I'd suggest replacing any special characters in file names plus signs, ampersands, spaces, etc with hypens or underscores. I took a quick look at the code but haven't yet figured where to implement functionality to rename the file.

Unfortunately this needs to be handled when the image tag is created. It's already been fixed in ImageField in #729584: Images with special characters in file name are not displayed. ImageCache will have to fix its own formatters.

I'd suggest replacing any special characters in file names plus signs, ampersands, spaces, etc with hypens or underscores.

That's exactly what the extremely frequently recommended Transliteration module provides.
