I have a tabbed menu built from the primary links list items. While not working on the menu or user settings, the admin/settings/headerimage link appeared when logged in as authenticated user. Heres what I've checked, done, and learned so far:
1. When logged in as admin it doesn't show on the primary links menu (works as designed as it isn't a Primary Link).
2. I've checked permissions for admin of the module and it was limited to only the admin role.
3. In the menu admin, the admin/settings/headerimage is not listed under my primary links menu.
4. I reset the menu item anyway to see if it would make a difference and it didn't.
5. I moved the menu item to another menu, saved, then moved it back again. No change
6. When selecting the menu item as authenticated user I am brought to the admin page.
6a. I can successfully edit the Block title.
6b. I can not get to the Configure Block link
6c. I haven't tried to delete the Image block yet
7. I logged out completely and still was given the same permissions as above.
Lastly, I have applied any of the Drupal updates that have been made available over the past couple of weeks. I will do that ASAP and report back if that makes any difference.
In closing, If there is anything else that would help diagnose the problem please ask and I'll do my best to help out.

sutharsan commentedApply the 5.x-1.1 version of Header Image. This should fix it. Mail me if it doesn't.

mikepetersonwi commentedThe update took care of the issue. Now, however, the module is not serving the one Header Image as setup. I'll have to look into that more on my own since I did make a lot of changes regarding permissions, etc. while troubleshooting the matter. I'll post back if I can't get it working.
