Submitting the form will result in an Ajax error

When I submit the form, I get the following error:

An AJAX HTTP error occurred.
HTTP Result Code: 404
Debugging information follows.
Path: /system/ajax

When I manually visit http://example.com/system/ajax, I do not get a 404 error.
