Cannot get module to use forward.tpl.php - it worked when I first implemented

For some reason, I cannot get the forward.tpl.php file to be used by the module now. I use the below code, but it isn't taking.

This was working a couple months ago when I first added the module. Now however, just doesn't work and if I can't get it to work the module is unusable.

Has anyone run into this? I have forward.tpl.php in my theme folder. Of course cleared cache. Bummer.
